What Is Hereditary Hemochromatosis?
Hereditary hemochromatosis (HH) is an inherited disorder in which the body’s iron reserves are improperly metabolized and excess levels are deposited in the tissues. Many HH patients are treated by undergoing “therapeutic phlebotomies or withdrawals,” in which blood is withdrawn for the purpose of reducing iron overload. Only patients with the hereditary form of hemochromatosis qualify for this program.
Current Food and Drug Administration (FDA) regulations allow HH patients to donate without an associated fee and more often than other blood donors, if necessary. If all safety regulations are met, the FDA also allows blood from those diagnosed with HH to be sent to hospitals for transfusion purposes.
Please read the following information carefully to understand how you can participate in the HH variance donor program. Any person diagnosed with HH may qualify if certain requirements are met.
There are two types of blood draws in this program – allogeneic or therapeutic. Allogeneic means that your blood can be used for transfusion to a hospital patient, while therapeutic means that your blood is not used for transfusion purposes.
Are You Eligible?
Please take a moment to check your eligibility to be an allogeneic blood donor. Whether your status is allogeneic or therapeutic, the following forms must be completed and sent to Carter BloodCare’s Donor Notification department.
- The HH patient must complete the Hereditary Hemochromatosis Acknowledgement Form acknowledging that he/she understands NO FEE will be collected for phlebotomy, no matter how questions assessing donor health are answered or how often phlebotomies are performed.
To begin your enrollment process, complete the Donor Acknowledgement Form below and forward to Donor Notification. This form is only required once for enrollment into the CBC HH Program.
- An Enrollment/Prescription for No-Fee Phlebotomy Form must be submitted by the donor’s physician stating hemoglobin limits, phlebotomy frequency instructions and disclosure of other health (heart/lung) concerns. This will be kept on file in our office. This form must be sent, emailed, scanned or faxed to our offices at least 5 business days prior to your first appointment, and must be renewed each year by your physician.
The Enrollment Form (physician’s form) needs to be completely filled out. Please do not alter or add to the form, as this will delay your enrollment. This form is valid for one year, or until an updated form is received.
This is a donor driven program; you and your physician will determine when you need to be drawn. A form marked for every 4 weeks will allow you to be drawn with at least a 4 week interval between donations. It does not require you to donate every 4 weeks.
The patient may either complete the top portion of the Enrollment Form below and forward it to their physician for signature or contact the physician and ask them to send a completed form to the contact stated at the top of the form.
Please also use the Enrollment Form if you are already enrolled in the HH program and are updating a current or expired form.
When we have received both forms you will be notified by mail that your enrollment is complete. However, if you would like to check your status before you receive the letter please call Donor Notification at (817) 412-5603.
As a member of our HH program, WE WILL ALWAYS OFFER PHLEBOTOMY AT NO CHARGE TO YOU as long as you need to be drawn. If for any reason you do not qualify to donate or become deferred, we can still draw your blood as necessary. If you do not meet the criteria or do not want your blood to be donated for transfusion purposes, please contact Donor Notification so that we may update your account.
Some severe cardiac or lung conditions may exclude you from being able to be drawn at Carter BloodCare. If a history of cardiac/lung conditions applies to you, please call Donor Notification at (817) 412-5603 or email at firstname.lastname@example.org. Donor Notification will be able to address any potential concerns, and may need to have additional paperwork submitted from your Cardiologist/Physician.
